If you have applied for a passport following the guidance above but have been instructed to contact us because there is an issue with your vaccination passport, please contact us by emailing enquiries.w97291@wales.nhs.uk

Provide as much detail as possible regarding your issue with the passport, including your full name, DOB and NHS number. Please allow 72 hours (excluding weekends) for us to investigate.

COVID-19 Booster Update

Cardiff and Vale University Health Board are currently delivering the COVID-19 booster vaccination to all those eligible.

If you are eligible, you will be offered your booster at least 6 months after your second dose. The Health Board are scheduling booster appointments in the order second doses were given.

When it’s your turn, you will be sent an invitation letter with your appointment details. Do not call your GP Surgery please wait to be contacted and help us keep the telephone lines free. If you need to cancel or re-schedule an appointment please call the Vaccination Booking Line on: 02921 841234.

Please note: you cannot walk-in for booster vaccinations, you will be contacted directly.

COVID19 Update: March 2020

As of 16th March 2020 open surgery will be suspended until further notice.
DO NOT COME TO THE SURGERY  
Please ring the surgery to be triaged and you will be offered an appointment as needed.
